Conversations that Count

We would like to invite people from your organisation to attend Conversations that Count Day on Wednesday 5 April 2017, which aims to raise awareness about advance care planning, to encourage people to think about, talk about and plan for their future and end-of-life care.
To mark day, the Health Quality & Safety Commission, in conjunction with Counties Manukau Health, is hosting a morning tea at the Auckland Botanical Gardens for people in South Auckland to learn more about advance care planning.
It’s an opportunity to hear from those who have prepared an advance care plan and the peace of mind it has given them and their loved ones. Health professionals will also speak about how treatment and care plans can support what someone’s advance care plan says is important to them.
There’ll also be lots of information to help people prepare their advance care plan.
Please note that this is a catered event and numbers are limited, so it is important people let us know they are coming by responding to the email address on the invitation by 20 March.
